What is this medication?

Pyridostigmine, Mestinon (US/CDN)/Regonol (US)
Acetylcholinesterase Inhibitor

Indicated for myasthenia gravis (oral only); military use (Soman nerve gas exposure); reversal of nondepolarizing neuromuscular blocking agents (injection only)

How does this medication work? 

Pyridostigmine, Mestinon/Regonol inhibits the breakdown of acetylcholine by acetylcholinesterase which is a neurotransmitter with various effects.

How should I take this medication? 

Myasthenia gravis: Oral: Immediate release:  60 to 1,500 mg/day, usually 600 mg/day divided into 5 to 6 doses, spaced to provide maximum relief.
Sustained release:  180 to 540 mg once or twice daily (doses separated by at least 6 hours)

Reversal of nondepolarizing neuromuscular blocking agents: IV: 0.1 to 0.25 mg/kg/dose.

How to Take: Follow instructions provided by physician/pharmacy label, take consistently at same times each day unless otherwise directed, store at room temperature.  For injectable dosage form administer as slow IV push. ER tablets should be swallowed whole

You Need to Avoid: Do not crush ER release tablet.  

What should I watch for while using this medication? 

Before starting Pyridostigmine, Mestinon/Regonol make sure your physician is aware of any allergies or medications you currently take, if you have a mechanical intestinal or urinary obstruction, cardiovascular disease, glaucoma, renal impairment, respiratory disease, are pregnant, or breastfeeding. Pyridostigmine, Mestinon/Regonol use has been associated with rare but serious adverse events such as excess cholinergic effects (salivation, sweating, urinary incontinence, muscle weakness, etc) and hypersensitivity reactions.

What if I miss a dose?

If you miss a dose of medication, try to take it as soon as possible. However, if it is almost time for your next dose, take only that scheduled dose. Do not take double or extra doses.

How should I store this medication?

Keep out of the reach of children at all times. Store at room temperature, 59 to 86° F (15 to 30° C). Protect from light. Keep the container tightly closed. Properly dispose of any unused medication after the expiration date.

What are the possible side effects of using this medication?

Twitching, hyperesthesia, xeroderma, abdominal pain, diarrhea, dysmenorrhea, urinary frequency, myalgia, neck pain, amblyopia, epistaxis
