What is this medication?

Adefovir Dipivoxil, Hepsera
Antihepadenaviral, Reverse Transcriptase Inhibitor

Adefovir Dipivoxil is indicated for treatment of chronic hepatitis B.

“Alert: Black Box Warning

Adefovir, Hepsera can cause severe acute exacerbation of hepatitis in patients who have discontinued anti-hepatitis therapy.  Always monitor hepatic function for at least several months in patients after discontinuation of anti-hepatitis B therapy.  In patients at risk of underlying renal dysfunction, nephrotoxicity can occur and as a result careful close monitoring of rental function is a must.  Lactic acidosis and severe hepatomegaly with steatosis, including fatal cases, have been reported with the use of nucleoside analogs alone or in combination with other antiretrovirals.”

How does this medication work?

Adefovir, Hepsera inhibits HBV DNA polymerase (reverse transcriptase) causing DNA chain termination which stops viral replication.

How should I take this medication?

Oral: 10mg once daily.

How to Take: Take with or without food.

What should I watch for while using this medication?

Before starting Adefovir, Hepsera make sure your physician is aware of any allergies or medications you currently take.   Prior to starting Adefovir, Hepsera there are a number of serious warning to be aware of including fatal lactic acidosis/hepatomegaly, acute exacerbation of hepatitis B may occur upon discontinuation, may cause the development of HIV resistance and has led to renal toxicity in at risk populations.  Current clinical hepatitis B practice guidelines do not recommend adefovir for initial use in the management of chronic HBV due to high rate of resistance with long-term use; other antiviral agents with a high barrier to drug resistance are preferred (i.e. tenofovir or entecavir).
