What is this medication?

Pirfenidone, Esbriet
Anti-Inflammatory; Anti-Fibrotic

Officially indicated for use in pulmonary fibrosis but is sometimes used off-label for skeletal muscle fibrosis in sports medicine.

How does this medication work?

Pirfenidone has been shown great promise as both an anti-inflammatory and anti-fibrotic in skeletal muscle.  Pirfenidone is believed to decrease fibroblast proliferation; decrease Type 1 collagen, fibronectin, and extracellular matrix accumulation in response to transforming growth factor β and platelet-derived growth factor.

How should I take this medication?

Skeletal muscle fibrosis (Off-Label)
Oral: Take as directed by the doctor.

How To TakeTake with food to decrease frequency of dizziness or nausea. Limit or avoid grapefruit juice.

What should I watch for while using this medication?

Use with caution in patients who require mental alertness 

Advise and monitor for weight loss during therapy

What if I miss a dose?

If you miss a dose of medication, try to take it as soon as possible.  However, if it is almost time for your next dose, take only that scheduled dose.  Do not take double or extra doses.

How should I store this medication?

Keep out of the reach of children at all times.  Store at room temperature, 59 to 86° F (15 to 30° C).  Protect from light.  Keep the container tightly closed.  Properly dispose of any unused medication after the expiration date.

What are the possible side effects of using this medication?

The following side effects reported are from short-term studies, frequency varies between different products:


Central Nervous System: Fatigue (26%), headache (22%), dizziness (18%)

Dermatologic: Skin rash (30%)

Gastrointestinal: Nausea (36%), diarrhea (26%), abdominal pain (24%), dyspepsia (19%), anorexia (13%), vomiting (13%), gastroesophageal reflux disease (11%)

Respiratory: Upper respiratory tract infection (27%), sinusitis (11%)
