What is this medication?

Phosphodiesterase-4 Enzyme Inhibitor

Indicated for treatment of Chronic Obstructive Pulmonary Disease (COPD).

How does this medication work? 

Roflumilast and its active N-oxide metabolite selectively inhibit phosphodiesterase-4 (PDE4).  Inhibiting PDE4 leads to an accumulation of cyclic AMP (cAMP) in inflammatory and structural cells important in the pathology of COPD. Anti-inflammatory effects from suppression of cytokine release and inhibition of lung infiltration by neutrophils and other leukocytes. Pulmonary remodeling and mucociliary malfunction are reduced.

How should I take this medication? 

Oral: 250 mcg once daily for 4 weeks, followed by 500 mcg once daily.  An initial dose of 250 mcg once daily is recommended for the first 4 weeks of treatment in an attempt to improve tolerability, however, this is not considered a therapeutic dose.

How to Take: Follow instructions provided by physician/pharmacy label, take consistently at same times each day unless otherwise directed, store at room temperature.  Administer without regard to meals.

You Need to Avoid: Significant drug interactions exist, requiring dose/frequency adjustment or avoidance so avoid starting any new medications without consulting prescriber.

What should I watch for while using this medication? 

Before starting Roflumilast, Daxas/Daliresp make sure your physician is aware of any allergies or medications you currently take, if you have hepatic impairment, are pregnant, or breastfeeding. Roflumilast, Daxas/Daliresp use has been associated with rare but serious adverse events such as neuropsychiatric effects and weight loss.

What if I miss a dose?

If you miss a dose of medication, try to take it as soon as possible. However, if it is almost time for your next dose, take only that scheduled dose. Do not take double or extra doses.

How should I store this medication?

Keep out of the reach of children at all times. Store at room temperature, 59 to 86° F (15 to 30° C). Protect from light. Keep the container tightly closed. Properly dispose of any unused medication after the expiration date.

What are the possible side effects of using this medication?

Headache, dizziness, insomnia, weight loss, diarrhea, nausea, decreased appetite, influenza, back pain
